The health and
wellness component of Central Dallas Ministries is Community
Health Services, which includes primary care medical
services, dental services, community-based care coordination and a
Class D pharmacy operated in partnership with CommuniCare (click here
for more).
After successfully operating its first health
clinic for many years, the program opened its second clinic at the
Preston Road Church of Christ in December 2003. That clinic, known as Christ's
Family Clinic, is now located in the Highland Park United
Methodist Church.
The program also
recently launched CoDE
(Community Diabetes Education), which is helping dozens of
suffering patients suffering to manage this chronic disease. The
results of this program, which are outlined here, are gaining
national attention in the medical world.
These services are
made possible through the strength of collaborative partnerships with
Baylor Health Care System, Inc., Texas Tech School of Pharmacy,
Parkland Health and Hospital System, the Dallas
County Medical Society and several other community
organizations, churches, and non-profits.
We look at our
patients as holistic human beings who confront both medical and
non-medical barriers that prevent them from getting healthy. Our goal
is to be the health and wellness home of our patients, to teach them
preventative care and measures to enable them to take control of
chronic diseases (such as diabetes and hypertension), treat their acute
health needs, and help them access more advanced care and support when
needed through care coordination.
